Disability
The State Universities Retirement System (SURS) provides disability benefits for eligible University employees. Applications for SURS disability benefits are distributed by Labor and Employee Relations.
To receive a SURS disability application, an employee must:
- schedule an appointment with Connie Foran at 217-333-3105 and
- provide a physician's statement describing the diagnosis and prognosis and stating that the employee must be off work for more than 60 days.
